I am allowing my horse to receive massage. I understand that massage is not a substitute for medical treatment or medications. For any medical issues that your animal might have, it is advised that you consult my veterinarian. I understand that any of the massage sessions are for the purpose of stress reduction, relief for muscle tension, general relaxation, and improvement of circulation and energy flow.

I am aware that equine massage therapists are not permitted to make medical diagnoses or prescribe medicine, and that spinal manipulations are not included in the massage. I understand that any information provided by the equine massage therapist is for educational purposes only and is not diagnostically prescriptive in nature.

I have informed the equine massage therapist of all my animals known physical conditions, limitations, medical conditions, and medications. It is my responsibility to update this information with the massage therapist and contact my veterinarian if my animal’s physical condition, limitations, medical condition, or medications should change.

I acknowledge that have the irrevocable and unrestricted right to use and publish photographs of me or my horse, or in which I may be included, for editorial, trade, advertising, and any other purpose and in any manner and medium; and to alter and composite the same without restriction and without my inspection or approval. I hereby release the Lisa Turcotte and her legal representatives and assigns from all claims and liability relating to said photographs.
